2022年10月10日 · What is TPOPP/POLST? The TPOPP/POLST initiative is designed to improve quality of care for those living with serious illness and/or frailty by translating patient goals and preferences into medical orders.
TPOPP is a useful tool for the understanding of and implementation of physicians’ orders that are reflective of the current medical condition and preferences of a patient. The orders are to be respected by all receiving providers in compliance with institutional policy.
Who Might Have a TPOPP Form? • Those who: -- Live with advanced progressive chronic illness -- Might die within the next year -- Wish to further define their care wishes • TPOPP is NOT appropriate for: -- A person with stable medical condition or disabling problem with years of life expectancy -- Anyone who does not want it
The goal of TPOPP/POLST is to improve the communication of a person’s preferences regarding selective treatments, thereby ensuring higher family/patient satisfaction and goal-concordant care.

2022年11月30日 · There is a new, updated form for documenting the treatment wishes of patients at the end of life. The formerly bright pink TPOPP form (Transportable Phy¬sician Orders for Patient Preferences) was modified to better align with the national POLST form (Physician Orders for Life Sustaining Treatment).
TPOPP is based on the communication between the patient or their legally designated decision maker and their physician and health care providers that ensures informed medical decision making. The TPOPP form travels to all areas of care: home, transport by EMS, long term care or any hospital setting.
TPOPP/POLST is a physician order set and as such does not replace Advance Directives but should serve to clarify them. TPOPP/POLST must be completed by a health care provider based on patient preferences and medical indications.
