Anti-Jo-1 antibody is most common antisynthetase antibody. Others include PL-7 and PL-12. Five additional antisynthetase antibodies have been identified so far, but there are much more rare and their implications are not well understood yet. Researchers are still identifying new autoantibodies of significance to myositis diseases.

We describe the clinical and laboratory characteristics of a cohort of European anti-PL-7 patients, and compare them with previously reported cases. This multicenter study of patients positive for anti-PL-7, identified between 1984 and 2011, derives from the EUMYONET cohort.

Antisynthetase syndrome is a rare autoimmune disease characterized by interstitial lung disease and/or inflammatory myositis, with positive antisynthetase antibodies (anti-Jo-1, anti-PL-7, anti-PL-12, ZO, OJ, anti-KE or KS).

Anti-Jo-1, anti-PL 7, and anti-PL 12 are the most frequently noted anti-synthetase antibodies; ANA and anti-Ro/SSA may suggest myositis-related ILD. Has severe and aggressive ILD as compared to the other idiopathic inflammatory myopathies. Often requires multi-modality immunosuppression

Anti-Jo-1, anti-PL-7, anti-PL-12, anti-EJ, anti-KS, anti-OJ, anti-Ha, and anti-Zo antibodies target aminoacyl-tRNA synthetases and represent antisynthetase syndrome. Anti-Jo-1 is the most common autoantibody seen in antisynthetase syndrome and is present in an estimated 70-90% of patients with Myositis who have associated Interstitial Lung Disease.

Anti-PL-7 antibody recognizes threonyl-tRNA-synthetases and was detected in 5-10% of IIM (1) and in 10-18% of ASS (3 - 6). Although ASS has common clinical manifestations, its relative frequency varies with the type of anti-ARS antibodies (4 - 6).

Anti-PL-7 is an 80 kD antibody against threonyl-tRNA synthetase that was first identified in the sera of myositis patients in 1984 . Anti-PL-7 antibodies have been recognized in less than 5% of patients with myositis, although the prevalence may be higher in Japan ( 2 ).
