Think that a lentigo is the same as a mole (nevus)? I’ve been told they are, and aren’t, by two different dermatologists. “Lentigos are superficial tan to brown flat spots on the skin which are often secondary to ultraviolet exposure and will occur on sun exposed areas,” says Kally Papantoniou, MD, FAAD, with Simply Dermatology in NY.

A lentigo is a clearly defined, pigmented macule that results from sun exposure. Unlike a freckle, it does not fade over time. Images below demonstrate the various possible forms.

A mole or nevus can look identical to a lentigo. Both a mole and a lentigo have the potential to develop into melanoma, a skin cancer that kills thousands of Americans every year.

Flat melanocytic lesions or freckles include ephelis, solar lentigo and lentigo simplex. These are distinguished by their histological features. The ephelis is a solar-induced macule with a normal number of melanocytes that produce increased melanin, which is distributed among keratinocytes.

A clinical diagnosis of lentigo maligna at an early stage is often difficult even for experienced dermatologists. Differential diagnoses would include solar lentigo, early lesions of seborrheic keratosis, lichen planus-like keratosis, pigmented actinic keratosis and melanocytic nevus.
