Stent angioplasty is a feasible and effective method of relieving LPA obstruction caused by an ADO II device. The Cook Formula stent is a premounted balloon-expandable stent that can be significantly overdilated to double its original size with virtually no shortening allowing for precise placement and minimal protrusion into adjacent vessels.[ 8 ]

Albeit infrequent, device-related left pulmonary artery (LPA) stenosis is a known complication of this procedure, occasionally requiring stent placement to relieve the obstruction. However, to date no real solution has emerged. LMB compression could be avoided with a vertical oval stent geometry. We developed a novel 3DRA-guided stent technique to perform oval LPA stenting safely in single ventricle patients at risk of or with pre-existing LMB compression.

We present a patient with tetralogy of Fallot who developed bilateral branch pulmonary artery stenosis following surgical repair that was refractory to balloon dilatation. Simultaneous stenting of both pulmonary arteries produced a good result.

Transcatheter stent deployment is an effective, yet high-risk intervention in pulmonary artery (PA) stenosis, associated with vascular injury and airway hemorrhage. In the US, the only covered stents for this application have been off-labeled use of bronchial stents (iCast – Getinge, Gothenberg, Sweden) and self-expanding stent grafts.
