For the HB, get use to it as a long range rifle, once you get comfortable with downing people from a distance you'll be more comfortable using it up close, i.e. your shots will be easier to land (grouping) head shots to down people quicker.

Best way to do it would be to have 1 person set their difficulty to Insane then have the other 3 set theirs to Casual. Then repeat for the next person until all of you have the achievement. Having all 4 undertake this incredibly painful and frustrating journey on Insane is simply impossible barring some sort of exploit or glitch.
