It indicates that there are two foci of pacemaker cells firing simultaneously: a supraventricular pacemaker (e.g. the sinus node) and a competing ventricular pacemaker (source of ventricular ectopics).

Capture beat occurs when a sinus impulse conducts through the AV node and depolarizes the ventricle (captures the ventricles) producing a QRS complex with normal duration between wide QRS complexes in the setting of AV dissociation.
