Cytochrome P450 2C19 (abbreviated CYP2C19) is an enzyme protein. It is a member of the CYP2C subfamily of the cytochrome P450 mixed-function oxidase system. This subfamily includes enzymes that catalyze metabolism of xenobiotics , including some proton pump inhibitors and antiepileptic drugs.

The CYP2C19 gene provides instructions for making an enzyme that is found primarily in liver cells in a cell structure called the endoplasmic reticulum, which is involved in protein processing and transport.

CYP2C19 genotype-guided drug prescribing has been proven in several trials to increase pharmacological effectiveness and safety. For example, CYP2C19 PMs have been observed to have a greater risk of cardiovascular events in patients receiving antiplatelet treatment with clopidogrel following stent installation.

The CYP450 enzyme CYP2C19 is at least partly involved in the metabolism of many drugs, but inhibitors of CYP2C19 will tend to have the greatest effect on drugs for which CYP2C19 is the primary pathway (eg, those with an asterisk in Table 1).
