McConnell’s sign is defined as right ventricular free wall akinesis with sparing of the apex. Typically this looks as if the apex of the RV is a trampoline bouncing up and down while the rest of the RV remains still. This finding is not sensitive, but in a small study was specific for an acute PE.

McConnell's sign, an echocardiographic finding of segmental right ventricular wall‐motion abnormality with apical sparing, is highly specific in acute pulmonary embolism and may guide rapid intervention when other testing is not feasible.

Multiple theories have been proposed regarding the mechanism of McConnell’s sign in the context of acute pulmonary embolism. Here, we present 2 patient cases in which McConnell’s sign was seen with right ventricular ischemia without pulmonary embolism.

"McConnell's sign", defined as RV free wall hypokinesis in the presence of normal RV apical contractility, has 77% sensitivity and 94% specificity for the diagnosis of acute pulmonary embolism, with a positive predictive value of 71% and a negative predictive value of 96% (8).

McConnell's sign is a distinct echocardiographic feature of acute massive pulmonary embolism. It is defined as a regional pattern of right ventricular dysfunction, with akinesia of the mid free wall and hyper contractility of the apical wall.

“McConnell’s sign” (McCS), described as hypo- or akinesis of the right ventricular (RV) free wall with preservation of the apex, is associated with acute pulmonary embolism (aPE). However, the sensitivity of McCS for the detection of aPE is limited.
