HOW CAN WE TEST FOR MENINGEAL IRRITATION? There is the Kernig’s Sign and Brudzinski’s Sign. In Kernig’s Sign, the patient is supine with the knee bent at right angles. As the leg as straightened, it causes leg pain. In Brudzinski’s Sign, when the neck is flexed, it causes flexion of the hips and knees.

Jolt accentuation of headache is a new and less well-recognised physical examination, which assesses meningeal irritation. It is judged as positive if the headache is exacerbated by rotating the head horizontally two or three times per second.

Horizontal rotation of the head at frequency of 2 rotations/second - exacerbation of pre-existing headache is positive test. Although a 1991 study [1] showed high sensitivity with this test, multiple newer studies have cast doubt on its sensitivity [2] [3].

Jolt accentuation is a physical finding that is believed to assess for meningeal irritation. Exacerbation of the baseline headache with horizontal rotation of the neck (2-3 times/second) is considered positive.
